Is the Friends of Costco Guild a non-profit?
The Friends of Costco Guild is a 501(c)(3) organization within the Seattle Children's Hospital Guild Association. The Seattle Children's Hospital Guild Association is a non-profit corporation, exempt from federal income tax. Friends of Costco Guild Tax ID number and an electronic copy of our W9 form is available upon request.

Who do I make checks payable to and what is the street address for delivery of payments?
All checks should be made payable to Friends of Costco Guild and sent to:

How do I sign-up to be a Sponsor?
The Friends of Costco Guild is pleased to welcome returning sponsors and new corporate partners. Sponsorship packets are automatically mailed to all previous sponsors in February. Companies who have indicated an interest will also be mailed a sponsorship packet in February. If you are new to the Children's Golf Classic and would like to receive a full sponsorship packet, please send your mailing address to: office@friendsofcostco.org. The sponsorship form can also be downloaded at Hole 5 on the left side of this page. Upon receipt of your completed form, an invoice will be sent via e-mail and your sponsorship will be confirmed with receipt of payment. Sponsorship forms can be submitted by e-mail at office@friendsofcostco.org or fax to 206-577-3966 or mail to Friends of Costco Guild; PO Box 19755; Seattle, WA 98109. Although there is not a firm deadline for sponsorships, we recommend signing up as soon as possible since some sponsorship levels fill quickly. (Several levels were sold-out by May in 2012.) Remember that sponsorships are only confirmed with receipt of full payment.

When do I have to have my team players' names in?
The complete team registration packet will be sent to all confirmed sponsors in late June. A completed Player Registration Form is required for each team and can be submitted any time prior to July 15. If you have player changes after your form has been submitted, changes can be sent via e-mail to office@friendsofcostco.org. Please be sure to include full contact information for new players. Costco player preferences can be noted on the Sponsorship Form and/or the Player Registration Form. All Costco players are placed based on sponsorship levels. We will make every effort to fulfill your request, however with the growing number of teams we may not be able to accommodate everyone. The final pairings will be available on Sunday, August 18 at Safeco Field.

How do I find out what course my team will play?
Course assignments will be provided at the Kick-Off Party on Sunday, August 18 at Safeco Field. If a member of your team will not be present, your course assignment will be e-mailed to your team contact upon receipt of your Player Registration Form.

How do I reserve rental clubs?
Club rentals are available with an advance reservation made through Rosanna Bellotti. Requests can be sent to office@friendsofcostco.org (please designate right or left-hand and men's or women's with your request). Due to the limited supply, we can offer only one set per group. (The scramble format allows for sharing clubs without slowing play.) Rental fees vary between $25 and $60 and must be paid in the Pro-Shop prior to play. (Please do not call golf courses for rentals. Reservations made directly with courses will not be honored.)

What is the proper attire?
Attire for both the VIP Reception on Saturday and the Kick-Off Party on Sunday is business casual (coat and tie not required). Proper golf attire is required on Monday at all courses and suitable for the Awards Party following golf. Please refer to Tournament Information (Hole 3 on the left side of this page) for complete description of Proper Golf Attire.
